Prostate specific antigen (PSA) is a protein produced by the cells of the prostate gland. Serum prostate specific antigen levels are low in men, however become elevated in prostate cancer patients. Because of this, the prostate specific antigen test is the most effective method for early detection of prostate cancer. However, total PSA levels can become increased by prostate infection, irritation and benign prostatic hyperplasia (BPH), and these can lead to false positive tests for prostate cancer.
PSA exists in three forms; free PSA, PSA-ACT (PSA bound to Alpha-1 Antichymotrypsin) and PSA-A2M (PSA bound to Alpha-2 Macroglobulin). Analysis of the ratio of total serum prostate specific antigen to free prostate specific antigen (non protein bound) gives a more precise test for prostate cancer. A lower ratio of free to protein bound PSA gives an early indication of prostate cancer. PSA is also used in forensic science to confirm the presence of semen.
